Update to Share 911 Reporting

Dear Clarkstown Parent and Guardians, 

The New York State Department of Health has required that if your child is scheduled to be in school on a particular cohort day, you must complete the Share911 Wellness Screening.  If, for any reason including illness and excused absences, your child will not be coming into school as expected, completing the Wellness Screening is still required and can be done by simply choosing the “Off-Site” option.

To facilitate the screening process, CCSD is employing Share911 Wellness, a simple to use mobile app that enables you to quickly confirm that your child doesn’t have a fever or other COVID-19 symptoms. To access the Share 911 Wellness app, follow these steps: 

  • Download the free Share911 Mobile app from the Apple Store or Google Play
  • Once downloaded, enter your email address (the email address should be the same one that CCSD used to send you this email).
  •  After entering your email you will be prompted to complete your Share911 account. 

Allowing notifications for this app, will enable Share911 to remind you to complete the wellness screening each day.  If your child does not complete the screening on a school day, the school nurse will contact you. Your child cannot attend school until the screening is completed. 

If you do not have a smartphone, you can complete your child’s screening on the Share911 website, www.share911.com or by using the screening form and send it to school with your child if there are no positive responses on a screening day. 

Daily Wellness Screening is a core component in our multi-layer strategy to keep your children and our CCSD community healthy and our school open. This is a process based on the honor system and we ask your commitment to fulfilling this requirement.  The wearing of face coverings, maintaining social distance and handwashing and sanitizing are other practices put in place throughout our schools.
