MANDATORY Student Health Screening
Dear Clarkstown Parents, Guardians and Staff,
NYS continues to require all students, staff and visitors to complete a wellness screening before entering any school building, school bus or the District Office. The parents/ guardians of students who have been attending in person instruction have been doing this, in most cases, using the Share911 Mobile App and should continue to do so.
For those parents whose students are returning to in-person instruction from the remote environment, you will need to set up your Share911 account so that you can complete the screening questions for your child/children each morning before sending them to school. Please watch this short (less than 2 minute) video for more details.
To access the Share 911 Wellness app, follow these steps:
- Download the free Share911 Mobile app from the Apple Store or Google Play
- Once downloaded, enter your email address (the email address should be the same one that CCSD used to send you this email).
- After entering your email you will be prompted to complete your Share911 account.

If your child does not complete the screening on a school day, the school nurse will contact you. Your child cannot attend school until the screening is completed.
If you do not have a smartphone, you can complete your child’s screening on the Share911 website, www.share911.com or by using the screening form and sending it to school with your child, as long as they pass the screening on that day.
