When your child will be absent or late:
Please call the nurse at (845) 624-3471 x4704 and leave a message on the answering machine.
If your child has any allergies, please notify the school nurse. We will notify classroom teachers/staff accordingly in the event there is a child with a severe allergy in a classroom that all must be aware of.
Head lice seem to multiply in the summer and fall months due to sleepovers at friends and sleepover camps. Please check your child's head frequently, starting at the nape of the neck and behind the ears. If you treat/are actively treating your child for head lice, please notify the school nurse immediately.
Medical and Dental Exams are required by New York State for all students in Kindergarten, First, Third, and Fifth grades and new entrants. New York State Law requires a scoliosis exam on all girls entering 5th grade. The "Annual Physical Exam K-12" form is linked herein, can be found online under the health services tab, or you may obtain a copy in the school's health office.
The policy of the Clarkstown Central School District regarding children taking medication during the school day is as follows:

P.E./ RECESS EXCLUSION/ CRUTCHES
Children may be excused for only one day with a parent's note. A physician's note is required for longer exclusions from injury, surgery, or hospitalization. A physician's note is also required to return to P.E. If your child's injury requires crutches - a "Crutch Permission Form" must be completed and signed by the doctor AND parent before returning to school. This form is linked herein, can be found online under the health services tab, or you may obtain a copy in the school's health office.
If your child has a rash that is itchy or oozing, please keep him or her home and contact your physician. A note from your physician is necessary for your child to return to school.
If your child has a throat culture done at the physician's office, please keep him/her home until you have the results. CCSD's policy is that a student can return to school after 24 hours on an antibiotic, no physician's note required.
If your child has a temperature over 100 degrees in the evening or morning before school and shows signs of an illness such as a cough or sore throat, please keep him or her home and seek the advice of your physician if necessary. A child should remain home until they are fever free for 24 hours WITHOUT fever reducing medication.
