How Do I Register My Child?

We are pleased to offer an online registration process using Forms & Flows (Operoo). This system allows families to securely complete and submit all required registration forms electronically, following the steps below:

Registration Request Form - Complete a Registration Request Form in eSchool for each child being registered today, to begin the process:

My family is new to CCSD and/or I do not have a Parent Portal account

I have a student currently enrolled at CCSD and I have a Parent Portal account
Forms and Flows Invitation - Within 24 hours you will receive an invitation from Forms & Flows with directions for logging into their system and your Forms & Flows Parent/Guardian Dashboard. This is where you will find the forms required for registration.
Welcome Form - Once logged into Forms & Flows, the first form you will see is our Welcome form. Shortly after completing this form, the additional forms required for your child's registration will be added to your Forms & Flows dashboard. See below for a listing of documents you may need
Forms Submitted - After you have completed all of the required forms, our Registration office will begin registering your child. If any additional information is required, you will be notified through the contact information you provided.
Complete - Once you have completed all of the required forms and your application has been approved, the registration office will reach out to you to make an in person appointment to finalize your registration application.

What is Forms and Flows?

How do I create a Forms and Flows account?

Once you complete the initial online registration request, a Forms and Flows account will be created for you. You will receive an email invitation from Forms and Flows Administration, similar to the image below, within a few hours as your request is processed.

This email invitation will include your username and a temporary password. Simply click the sign-in button in the email, and you’ll be guided through setting up your new password and choosing your preferred language. This video illustrates the steps.

If you don’t see the invitation email within 24 hours, please reach out to the Registration Office—we’re happy to help!

How do I navigate the parent/guardian dashboard?

The Forms and Flows parent/ guardian dashboard is made up of simple sections created for easy navigation. Watch this video for a quick overview of read the bulleted list below.

Forms & Flows Parent Account Basic Navigation

When you log into your Forms & Flows Account, you will see your Forms Hub page. It is defaulted to the Forms Requests page. Anytime you want to navigate to another page, you can always click on any of the choices on your dashboard menu.

Globe Icon - sets your language preference.
Help - Link to Operoo Knowledge Base for help articles.
Profile Icon - Where you can find your account settings, managed linked accounts, switch between legacy Operoo and Forms & Flows, set your preferred language, and log out of your account.
Form Requests - Any forms that the school needs you to complete will appear here. Click on START FORM to fill out the form. Once you have completed the form and are ready to submit, click SUBMIT RESPONSE. If you are not ready to submit, you can always click SAVE FOR LATER and come back to complete it when you are ready.
In-Progress - Any forms that you have marked SAVE FOR LATER will appear here. Just click the CONTINUE button on the form that you want to complete.
Completed Forms - A list of every form you have submitted. If you need to review your submission, click VIEW FORM for the form you want to review.
Form Center - This is where you will find all of the Self-Service forms available to you. Click STAT FORM on any form you want to complete.
My Students - View of personal student information and eForm responses.
Chat - Chat directly with a Support specialist

What if I have more than one child in the Clarkstown Central School District?

If you are registering more than one child either simultaneously or a different times, a separate Forms and Flows account will be created for each child during the initial registration process. You will receive an individual email invitation for each child, each with its own username and temporary password.

After you have completed the registration process for all of your children, these accounts can be linked so that you can access all of your children’s information using a single login. This video illustrates the simple steps for linking your accounts.

Is there a Forms and Flows App?

Yes there is! So happy you asked. Not only does the Forms and Flows app provide a convenient place for you to complete forms and applications, but it also allows you to upload documents directly to the app using your mobile device's camera as a built in scanner. Nice!

Look for the Forms and Flows, SchoolStatus App in either the Google Play or Apple iTunes store and download it today.

What documents are required to register my child?

Some or all of the documents listed below are needed to complete your registration. As you work through the forms on your Forms & Flows dashboard, you’ll be prompted to upload each document directly into the secure system.

For convenience, many people find it easiest to use the Forms & Flows mobile app (see previous question) and take photos of their documents using their phone’s camera. You’re also welcome to upload them through the Forms & Flows website—whichever works best for you!

Parent/Guardian driver's license or government/state-issued photo ID
Deed, mortgage statement, property tax receipt or current lease/rental agreement (only ONE)
Most recent utility bill
Child's original birth certificate
Child's most recent physical
Child's immunization record
Most recent report card or transcript
If applicable, please provide documentation pertaining to custody/visitation of the child(ren).
