During Kindergarten Screening, parents will access this interview form:

School trips are coming up and there are questions about parents as chaperones.
- If the school determines they need additional chaperones for a given trip, and this can not be covered by CCSD staff, a parent may serve as a chaperone.
- In order to chaperone, you will complete the attached document which includes fingerprinting and notary. Please return to Lakewood as soon as possible, as BOE approval is required. This is not a process that can be rushed based on an upcoming trip date.

- If the school determines that volunteers are needed to support the logistics of a class trip, the number of volunteers will be determined and communicated in advance.
- In order to volunteer on a class trip, you must be checked in via Raptor system at the front desk within 2 weeks of the trip date. A list of approved volunteers will be sent to the district office. No additional paperwork is required.
