Special Health Needs and School Physical
SPECIAL HEALTH NEEDS:
If your child has any special health needs or problems, this should be communicated to his or her school nurse. This information is kept confidential and it can be very important to a successful experience in school.
SCHOOL PHYSICALS:
The New York State Education Law requires Physical Examination for pupils upon entrance to school and routinely at grade K-1, 3, 7 and 10.
If you child has a yearly examination by your family physician, please ask him/her to complete our school forms (available at all Health Offices).
The physical examination should be completed by October 15. We would appreciate having the form returned to the school nurse by this date. After this, the school physician must examine all pupils in the grades mentioned above for whom we have no record of the family physician's report.
Regular dental examinations and care by your family dentist are encouraged. The dental section of the annual physical examination form may be detached and returned to school whenever dental work is completed.
SCREENING:
Students are screened annually for visual acuity and hearing. Students ages 8-16 are also screened for scoliosis according to state law. Parents will be informed of any abnormal findings.
