Physical Education Exemptions and Medication in School
Physical Education Exemptions:
A parent may write a note excusing a student from physical educations for three(3) days. Students needing to be excused from physical education for longer than three (3) days need a doctor's note. A copy will be given to the physical education teacher, classroom teacher and playground coordinator. The original will be kept with the cumulative health record.
Medication in School:
Every effort should be made to administer medication at home, as it does represent a disruption in the student's day. However, if your physician feels that medication is necessary during the school day, please submit a completed form which can be obtained from the health office before medication is sent to school.
A new form must be filled out for each change of medication and renewed each school year. State law does permit administration of medicine during the school day but only with written directions from the physician and parent. [PLEASE NOTE: THIS INCLUDES ALL TYPES OF MEDICATION; OVER THE COUNTER DRUGS (ASPIRIN, TYLENOL, COUGH SYRUP) AS WELL AS PRESCRIPTION MEDICATION.] The medication must be in the original container, or a duplication professionally labeled by the pharmacist for this purpose. The parent or guardian must assume responsibility for have medication delivered directly to the school health office. No student should have prescription or non-prescription medication in his/her possession while at school
