New for 2006-2007 School Year
There is a change in the physical and dental requirements.
New York State Education Law requires that all new students and those entering  kindergarten, 2nd, and 4th grades have a physical and dental exam. Forms will be sent home in the report card envelope in June.
Please return  forms in September.

When not to send your child to school Please remember to notify the school when your child is absent.

Dear Parents and Guardians,

As healthcare practitioners caring for your children in the Clarkstown School District, one of our main goals is to aid in the prevention, control and spread of all communicable and transferable illnesses, diseases, and infections.

Therefore, to help maintain these goals the district nurses are providing information that you will find useful.

bulletIf your child is complaining of nausea, ear pain, a sore throat, or lack of appetite, chances are he/she is ill.
bulletThe following guidelines will help you make the decision to keep your child home.

 

bulletPLEASE DO NOT SEND YOU CHILD TO SCHOOL IF THEY:
bulletAre awaiting *"strep throat" test results
bulletAre complaining of a sore throat and it appears red.
bulletAre complaining of nausea, vomited the night before or in the morning
bulletHad or had a fever of 100 degrees or more the night before or in the morning.
bulletHave a suspicious rash.
bulletHave had a persistent cough and congestion.
bulletHave red, draining, itchy eye or eyes

*If your child is tested for strep throat, they should remain home while awaiting results of a throat culture. You must have your child on medication for 48 hours after having a positive strep culture. If your child returns to school in less than 48 hours, you MUST send in a doctor's note stating that your child is NOT CONTAGIOUS  and may return to school.

When your child is going to be absent Please call the Nurse's office prior to to start of the day of the absence at 639-6350, you will be prompted to press 4 for the nurse.   You can leave a message on the Nurse's answering machine between the hours of 4 P.M. and 7:45 A.M. Kindly state the student's name, grade, teacher, days absent and reason of absence.

Following an absence from school: The child is required to submit a written excuse on the day of return.  The note should be written and signed by the parent/guardian and include the dates of and the reason for the absence.

When a child is ill in School:  We request that the emergency contacts person that is listed on the form resides locally for the benefit of the child. It is most distressing for a sick or injured child to have to wait for someone to come to school from out of the immediate area.
